Medicaid citizenship documentation

Effective September 25, 2006 the federal government has put new requirements in place for Medicaid eligibility. 

To receive (or continue to receive) Medicaid coverage, US citizenship must be verified

This means we must see and make a copy of ORIGINAL documents that prove this (you can’t bring us a copy). 

We have to do this for everybody in your household that receives a Medicaid card UNLESS that person receives

SSI or Medicare.

IProvide one of these original forms:

  • U.S. passport
  • Certificate of Naturalization (N-550 or N-570)
  • Certificate of U.S. Citizenship (N-560 or N-561)

If you do not have one of the forms listed above, then we need one item listed below from each category below :


Birthplace/Nationality Document Identity:

  • U.S. birth certificate 
  • Certificate of birth abroad (FS-545 or DS-1350) 
  • Report of birth abroad of a US citizen (FS-240) 
  • A hospital record of birth 

Identity:

  • State-issued driver’s license or state ID
  • Government issued ID
  • US Military ID or record
  • School ID with a photo

For children under the age of 16:

  • School record or report card
  • Childcare or nursery school record
  • Affidavit made under penalty of perjury by parent or guardian

 

Even if you have given us copies of birth certificates in the past, we need to see the original documents once again and make a new copy so we can verify that we have seen the original.
